DTI or SEC/ Copy of Business Registration or Mayor’s Permit (if business owner)

Individual ITR or form 2316 (applicant)

Bank Certificate and Bank Statement (applicant)-should indicate the amount of money you have in the bank and the date it was opened, must be original or certified true copy; last three months

NSO Marriage Contract – for marriage couple applying for visa/for married applicant

Copy of Senior Citizen Card (if retiree/Senior Citizen)

For Housewife – Documents of Husband, Employment Certification or DTI or SEC/Copy of Business Registration or Mayor’s Permit (if business owner), NSO Marriage Contract/ Recent ITR/ Bank Certification, Affidavit of Support/ if husband is a retiree, attach Senior Citizen ID of husband and submit Bank Certification and NSO Marriage Contract/ Affidavit of Support

If minor ,DSWD Clearance and affidavit of support ( notarized,signed by parents/supporting person +financial docs)

If Student – Certificate of Enrolment/ NSO Birth Certificate/Documents of Parents - Affidavit of Support + School ID; leave of absence (if trip is during school days)
